APPLICATIONS:

1. All information must be provided. For example, if you do not have a business address, enter the word "NONE" in the blank. Physical Addresses must be given... P.O. Box numbers are not acceptable!

2. Please be certain to provide your Florida Driver's License Number, where requested, if you do not have a Florida Driver's License you must provide a photocopy of one of the following: Florida ID Card, Florida Voter's Registration, Homestead Exemption, Income Tax Forms, or a Sworn Affidavit of Florida Residency.

3. Declaration of Domocile - If you are not a U.S. citizen, you must obtain a recorded Declaration of Domicile from your county court house. (GreenCards, Alien Cards are not acceptable)!

4. Affidavit of Character Section: - The Affidavit of Character must be completed by an individual who is not your relative, and who has known you for one year or more. The Affidavit must be signed and the name of the affidavit must be printed or typed underneath the signature.

5. Bond Form
   
   1. Please sign the bond form only where indicated: "signature of applicant."
   2. Our office will complete all other information required on the bond forms. Completion of all "blanks" on the application is required and will speed up processing.
