Please sign and print your name where indicated on the Bond Form. This form is required in addition to your Application Form for new or renewing notary appointment.

When you fill out your Bond Form, please read the Application Checklist.

Please mail your Bond Form (required), Application Form (required), and if applicable, Certificate of Completion (from our educational web site) to our office along with your payment $95.00. We accept personal & company checks, money orders, and purchase orders made payable to Florida Notary Association, Inc.
