Please fill out the Application Form and be careful to provide all information requested to avoid unnecessary delays in the processing of your notary appointment. A friend who has known you for at least one year should complete and sign the "Affidavit of Character" section.

When you fill out your Application Form, please read the Application Checklist.

Please mail your Application Form (required), Bond Form (required), and if applicable, Certificate of Completion (from our educational web site) to our office along with your payment $95.00. We accept personal & company checks, money orders, and purchase orders made payable to Florida Notary Association, Inc.
